Patient Information

New Patients

The doctors consulting from Holland Park Family Medical Centre welcome new patients and ask that their new patients arrive at the Practice 10 minutes before the appointment to allow time to complete a New Patient Information & Consent Form.

Phone calls / messages to the Doctor

Phone Consultations are regular consultations and usually fees apply. If you have a message for your doctor, or wish to speak to them, you can pass this message through our Reception staff.

Your doctor may not check messages until the end of a session or at the end of the day. If you feel that you have an urgent matter to discuss, please inform reception so that appropriate arrangements can be made by your doctor.

Generally, your Doctor will return your call accordingly or request a message to be passed back to you.

Billing

The doctors consulting from Holland Park Family Medical Practice provide private billing appointments with Bulk Billing for children 0 – 5 years. They may have discount rates for Students from 6 years, Health Care Card holders and where bulk billing occurs it is at the discretion of the individual doctor.

Your doctor’s fees are payable at the time of consultation by cash or EFTPOS.

Your Medicare rebate can be directly deposited into your account at time of payment. Please ask at Reception for a full Schedule of Fees charged by your doctor. If you have been booked for a special service such as Mirena insertion, Implanon insertion, or skin cancer surgery, please check with Reception for the out of pocket cost which may be charged by your doctor.

Prescriptions and Referrals

In the interest of better managing your health, prescriptions and referrals require an appointment with your doctor. This is a legal requirement. If you have prearranged this with your doctor, and it is necessary for these to be supplied outside an appointment, an administration charge of $20 applies.

Non Attendance

We understand that it is sometimes necessary to change your schedule. Out of consideration for others who are seeking appointments with your doctor, we kindly ask that you give your doctor a minimum of 2 hours of notice if you wish to change or cancel an appointment. Your doctor may charge a fee (non-claimable) for non-attendances or repeated failures to give sufficient notice of cancellation of appointments.

Results notification

Results are only given by doctors unless otherwise arranged. Lengthy discussions regarding results will not take place by phone. This is a legal requirement. Please make an appointment with your dDoctor to discuss your results. Your doctor may phone you to discuss results if prearranged.
